A new online form will replace the existing Word document. The form is designed to collect the information needed to be able to respond to incidents in a more timely and effective way.
All potential personal data security breaches should be reported separately as soon as they are discovered. If for any reason you are unsure whether an issue constitutes a personal data security breach, please still report it.
The current MS Word Personal Data Breach Reporting Form (PDBRF) document will be replaced by a link to the new online form on the Data Protection website.
